Services

“All hospice care is palliative, but not all palliative is hospice.”

  • Hospice Care

    TREATMENT: provides care for the terminally ill, most often introduced in the last 6 months of life, focused on improving quality and comfort of remaining life

    ELIGIBILITY: love one must be certified as terminally ill, love one must agree to no pursue curative measures, love one must elect to pursue hospice care with specific hospice organization; if the physician has not mentioned hospice, then the patient and family can discuss the possibility of the hospice with the physician.

    CARE LOCATION: generally home-based, freestanding hospice center, hospital, nursing home and other long-term facility

  • Palliative Care

    TREATMENT: ca be used alongside curative and life-prolonging measures, given to those with terminal and non-terminal illness, can begin at any stage of the disease

    ELIGIBILITY: loved one must request palliative care referral from physician

    CARE LOCATION: carried out in an institution such as hospital, nursing home or extended care facility

Admission Process: Admitting a patient to the hospice program is a simple process for the attending physician, discharge planner or social worker. Just call our office, and a nurse will visit the patient and family at home or in the hospital. Information about the program will be presented, and if the patient or family elects the program, the nurse arranges for start of service.

Payment Options: Medicare/Medi-Cal, health insurance/HMO, private payer

  • Medical Director

    Certifies the terminal illness with the attending physician

    Leads the interdisciplinary team in the development of a plan of care

    Provides consultation to other physicians regarding hospice care

  • Attending Physicians

    Co-certify the terminal prognosis

    Assess patient needs, manage symptoms, prescribe treatment

    Direct and approve the plan of care

    Coordinate care with the interdisciplinary team

  • Nurses (RN and LVN)

    Assess patient/family needs

    Develop plan of care to meet identified needs

    Coordinate team visits and ensure implementation of approved plan of care

    Coordinate care with attending physician, primary caregiver and/or care center staff

  • Hospice Aides

    Provide direct personal care to the patient

    Provide comfort measures, reports identified needs to Case Manager

    Provide emotional support to patient/family

  • Chaplain

    Assess patient/family for spiritual needs

    Develop plan of care to meet identified needs

    Provide bereavement support

    Provide consultations to community clergy

  • Social Worker

    Assess patient/family emotional social and financial needs

    Develop plan of care to meet identified needs

    Provide direct counseling

    Refer patients/families to appropriate community services

  • Volunteer

    Provide companionship, support to patient/family

    Provide needed non-medical services

    Provide support at time of death and during bereavement

  • Additional Therapy/Counseling

    Provide physical, occupational and speech therapy to develop an individual plan of care to meet their identified needs
